yeah, replace it...
horns are not deisgned as well as they should be, an more often than not, they are left hanging out in front of the radiator, and all it takes is one rock to smack it just right, and dent it a little, or even moisture can get into it, and freeze, causing it to expand... when it does, it forces the wire inside to make ground, and the horn to sound.
the only solution is to replace it... you can get replacements at any auto parts store for about 20 bucks.
