harmonic balancer keeps getting loose

Default harmonic balancer keeps getting loose
Montero sport 02' 3.5L
The bolt keeps falling out how can I fix this problem, I put lock-tight and the correct torque I also replaced the harmonic balancer for a new one.. what next?
Help!
