Just got some error codes off my car by connecting pin 1 to pin 4 on the OBD connector under the drive side dash (black connector) and turning the ignition switch on. The codes that came up were as follows:

Can anyone help decipher these codes?

My car has 244k miles and I started having a problem when the car is cold. When starting out, I put the car in drive and the car pulls forward, then it feels like the car comes out of gear (RPMs go up) and then it will jump back into gear and the car will jerk forward. When I come to a stop sign and press the brake pedal, the car seems to come out of gear again (RPMs go up slightly to around 2000) and then it will catch. To stop the car from jerking so violently, I started putting the car in neutral when the tansmission seems to come out of gear. I then put it back in gear and it will catch. Just recently, my car started going into safe mode (neutral light blinking). I turn the car off and back on and I can continue driving. All this happens in about 2 miles. Once the temperature in the car gets to the normal operating temperature, everything seems to operate as it should. The problem is just getting worse. Any help would be appreciated.
