Default inspection time fuel trim code

I normally run a down pipe w/ no cat. except for inspection. bolt on stock pipe w/ cat, reset ecu, inspect, and then change pipes.
I changed to stock pipe this year, reset ecu to get light off, and now i'm getting a Fuel Trim code.

additional info. Car is run hard. upgrade turbo fmic blow off exhaust intake blah blah. never got to installing fuel inject. fuel pump, and safc. I dial boost way down during winter or a get major fuel cut when I get on it.
Car sat for the winter when pulled out for summer. only boosting 10psi or so. turned up boost controller but cant get much more then 12psi. checked houses for leaks, cracks, and so forth
Hope for your help please let me know if there is any other info you may think relevent.
Car needs inspected... where do I start?
Any help would be great

Default exhaust hole

just inspected entire apexi exhaust system and found hole about the size of the bottom of a soda can.

Could this be the cause of the code?
