end 04 heat/ac climate fan

On my 04 Endeavor, the heat/AC fan has acting crazy. Working for a long time, then not working for a while. Could it be in one of the relays? if so which one? Thanks in advance.

Some times mine freezes if its on for a long time, i have to turn it off and then restarts its fine.
