Is my alternator going out?

Is my alternator going out?
I have a 97 Montero LS with 125,000 miles on it. It has been an incredible vehicle for me. Then just 2 days ago some warning lights began to glow.
This just started without any indicators. The voltage gauge went from indicating 14.5 volts when driving at around 2000 rpm to just right at 12 volts when coming to a stop or slowing down. The A/T light and Brake warning came on very dim as well as the battery warning. With my volt meter at the battery I read 14.5 V with the engine started but without anything on. When I turned on the AC and turned the headlights on it went down to 12.5 volts at the battery.
Any thoughts out there in Montero land?
Thanks
