In that case the "perfect storm" is not such a rarity. I've actually seen the wheel come off the car as it was driving down the street. It is a weird feeling to see a wheel skip along the street past you. On a separate occasion, a guy i know had it happen to his car. Additionally, I've seen many cars parked on the side of the road with the obvious problem that the wheel came off and they skidded to a stop. So, it doesn't take a perfect storm. Failures happen and if you do it wrong it happens more often.

Never in my life did i have a problem removing a wheel that had it's lug nuts torqued to spec.
