It would take a "perfect storm" of events to cause all 4-5 lug nuts to come off at the same time to cause the loss of a wheel while driving.
In that case the "perfect storm" is not such a rarity. I've actually seen the wheel come off the car as it was driving down the street. It is a weird feeling to see a wheel skip along the street past you. On a separate occasion, a guy i know had it happen to his car. Additionally, I've seen many cars parked on the side of the road with the obvious problem that the wheel came off and they skidded to a stop. So, it doesn't take a perfect storm. Failures happen and if you do it wrong it happens more often.

I cant stand the idiots who use torque wrench to install my wheels if I get them rotated or new tires cause then I can't hardle loosen them again.
Never in my life did i have a problem removing a wheel that had it's lug nuts torqued to spec.
